1: Bull Durham is a company that puts out chewing tobacco these days.

2: The Market Street Railway Co. (MSRC) was the company that operated San Francisco's cable cars and trolleys. (Trivia: SF's cable cars are the only National Landmark that moves.) The "Samuel" on your token would be Samuel Kahn, who was an MSRC president. The MSRC is now a volunteer non-profit organization that works to preserve the city's trolley past, so your token may be of particular interest to them if it's in good shape. You can contact the MSRC thru its website at www.streetcar.org.

3: No idea, altho I remember the word "KOMBO" being used in the bindery trade some time ago to refer to a machine that did page binding.
