Could this have happened at the mint???

There is no way this could have happened at the mint. If you are interested in what mint errors are, I would suggest learning everything you can about the coin manufacturing process. That, alone, would help you identify both mint errors, and post mint damage with greater accuracy.

Time for more coffee.

Found a few like those myself, pretty sure they're PMD. One way I found to figure out, look on the other side to see if theres any deformations that would correspond. Still, definitely a neat find.
