There are no design elements missing, so it's not technically off-center, It has probably just gone through a washing machine or something, which would explain its appearence, and it's 40 years old, which would explain the lack of detail. I'm sorry, It's not an error. :-[

From the Redbook-

"The designer's initials were added below the bust in 1966, and the dies were further remodeled to strengthen the design in 1971, 1972, 1977 and 1982."

Would explain some of the obvious differences in the details. It is slightly off center, but this is common.

Thought I had something.

Sitting on my desk mixed in with all my other coins it stands out, it looks so different than the other coins.
