Extra metal or air bubble trapped under the cladding. Due to the uniformity of the dome, IMHO it is likely an air bubble. As far as the Double Ear, it appears that there is slight Machine Doubling behind Roosevelt's ear and neck and on the Southeast and South side of the "D" Mint mark, the Date and possibly on "IN GOD WE TRUST".
