The one here in Rochester,MN is Cheap Charleys. Little place were there is not a seat that doesnt have a hole in it from ware. None of the dishes, cups, or flatwere match. One of those places that the food is good and cheap, but you don't want to look in the kitchen...Ever.

Hey, I've been there Jim, and indeed had the garbage plate! (the friend I was visiting in that area insisted upon it). Very good stuff. Their food has some character, none of those bland undercooked home fries and such.
But closer to home I'd say The Friendly Toast in Portsmouth NH.

I have eaten there many times. I don't think there is one coffee cup that matches another in the entire place.
Good cheap food though.
For breakfast I like the Canadian Honker. Two eggs, potatoes, toast/jelly, choice of meat, and coffee all for about $5.
You can't make it at home for much less than that.

Not far from me is a greasy spoon called....FAT BOY'S BACKYARD BBQ & MORE. Best BBQ, and fries I've had in years, and they peel and slice their on fries. :P
