Counter, help to find relatives

Hi, everybody! This counter with a note was found in the wood in Belarus about the city of Slutsk. I know that your forum is visited by people from the different countries.
It is written in my opinion on Cheshky or Slovaksky language but precisely I didn't manage to define so.
Excuse if placed a subject not in that section of a forum.
I hope for your help for transfer of this thing to rozhstvennik. Excuse if I write with mistakes.
