I say '45 because of the numeral's relative size in comparison to the '4'. the number 8 on a '48 is larger than the 4 much like the number 9. as far as an error, I say no. I believe that it was a weak strike, which is fairly common, and now exaggerated by the deteriorated condition of the surface of the coin.
