This is one of the coolest relics I've seen posted here in a while.

Here is one older than yours but the only one I could find with "ELEVAT POLI" on it:Epact: Astronomical Compendium signed by Christoph Schissler, dated 1566

This not like yours but it is marked:

sundial marked for 'ELEVAT. POLI 48'.

The question has been asked about whether the gnomon folds or is removeable. If not, I would argue that the piece is not a "pocket piece". Possibly part of a set or compendium?

Glad to have helped - again that's a very cool item! I'm okay with identifying the function of some things, but I shouldn't even attempt to guess their age. I'll have to defer to others more knowledgeable than me. Keep working to find out more about it - I'm sure there's more to it.
