From what I have experienced the open mouth also means that the cave, shaft, tunnel or cache area was left open and accessible.
The opening may be disguised and you may pass within a few feet of it and miss it all together.

may sound stupid but left open means,just open or lightly filled or covered not under 6ft of dirt? thanks brent

Brent,
Open means open.
There are signs for tunnels or caves / shafts that are backfilled a short distance. when you find them you will also need to know that they are blocked with a log block to keep the rest of the tunnel clear. if you realize this point in a hunt, you're a pretty lucky guy!
