    Looks like it may be from a field medical kit. Back then there were no pills or tablets to take. Everything was herbs or powders. This may have been a tool to scoop medical powders from containers and mix them with the flat blade portion for a special blend of powders.

    Not so sure about the fan base because there is not hole through the center, just a depressed dimple. How about an inkwell cover. Inkwells were usually glass but may have had a cover to dress it up a bit. If so, quill would have been inserted into the angled tube. don't know, just a guess.
