No question it is a cufflink. Best guess... Turn of the 20th C, give or take a few decades - child eating watermelon.

Wouldn't disagree with the dating... but it looks to me like a Japanese geisha wearing a kimono, and the crescent is just ornamentation.

That certainly is a possibility. Although, traditionally, Japanese women cover their smiles with the hand. I will admit the "watermelon" looks more like ornamentation. It was just my first impression...
