Well, it appears to be from an elementary school in Perugia, Italy and has the coat of arms of that city; http://en.wikipedia.org/wiki/Perugia

It is possible that your grandfather received this pendant when he completed his 5 years of schooling??

Thank you both for the information already given. He went to school in Illinois, so I know he didn't receive it for schooling. He was a missionary and I know he made many trips to South America, but am unaware of any trips to Italy.
