D and P are mintmarks
D=Denver
P=Philadelphia

Square?

You may be seeing a filled "D" Denver mint mark or possibly a fat "S" for San Francisco

Yeah, that is just a D mintmark. The die that is used to strike the coin chipped, so that metal flowed inside of the D.
Not too uncommon...
