I'd say 2 in How many gradings have they done over the Years :laughing7:
in which case there may be an 1940 p case with the 1948-D in it somewhere.

however I would think the original owner/submitter would have complained.

I agree! The original coin was cracked out and replaceed with the 1940-P Jefferson Nickel which was struck with severely fatigued and over used Dies. Aside from what appears to be Full Steps, note the weak strike on the rest of Monticello and especially the severe starburst effect on the Reverse and to a lesser degree on the Obverse which point to this. Even not accounting for the minor dings, nicks and scratches on the coin, with the starburst alone, the coin would never get an MS-65 Grade with or without Full Steps.

IMO, much more likely scenario is that the grading service made a typo on the label. It is also possible that the wrong coin was placed in this holder and that the 1948-D ended up in another incorrect holder.

Look up the 1940-P in the same condition. It is technically worth more money.

WHY would someone go to the trouble to crack it out and replace it with a more valuable coin? Highly unlikely...
