For sure, with out a doubt, it is not hand blown(I blow glass). From a mould. most likely 19th to 20 century. Earliest mould glass was 19th century in the us.

Was removed from mold too soon. = Reason form is "off".

OR... was victim of a fire.

Would never have survived the fire, i agree was removed from mould early or slumped during annealing.
