If your MD has no volume control, you need to add one inline.
If you are interested in higher end in-ears, these are the best at blocking outside noise and providing the best quality sound for the money Etymotic Research | Earphones
If you are interested in going wireless to the MD these are the best for the money and provide a wired sound quality without the wires or delay. I own and use both. Note you do not need the inline volume control with the transmitter it has one built in.

One other option that comes with that transmitter which is very useful, is a clip on lapel microphone. The mic can be clipped to its wire and the input has its own level control and can mixed with the MD output. This lets you hear your surroundings with your phones on.. (pinpointer, Dogs, people etc.. ) If you are not using the mic the mic input should be set to 0 to eliminate the hiss caused by no input on the line..
