It looks like a Lost Lake that had a bad spot they removed. I’d call that a pseudo-bifurcate. Typical great early archaic workmanship on the blade. Dognose, do you have any white flint sources in IN? It looks like typical Burlington to me.???
 

Jeffersonville flint, can be an off white, but often has lines or bands in the flint.
Laural chert is an off whitish yellow.
I do get a lot of burlington here.

That’s a great point. Haven’t found anything with deep notches like that yet.
