Victorian picture hanger (rail hook), hooks on a moulding. Not the more common design, but I think we've seen them here with good supporting evidence.

Thanks! Had never heard of it before. You have to wonder how it ended in the middle of nowhere. Looked at a bunch of pictures of them since your post. You are right, seems to be an unusual design. Most of them have two hooks. One for the moulding, one for the picture/frame. Mine only has the moulding one. Different way to attach the frame I guess. Anyway, thanks for identifying it.
