the buttons,the 1 w/2 holes does not have a back,the star button has a square shank all rusted,the spoon is stamped MED DEPT USA,the tap age?

I think the spoon is from the Medical Department of the United States Army, and is pre WWII .. probably the 1920's era, but I could be wrong.
