I think it's a plug designed to be put on the cleanout fitting to a sewer line. I have one like it downstairs. Keeps the smelly out.

It looks like a barrel bung. The threads appear corroded.

I just looked it over with a loupe and see nothing resembling threads, but i do see fine lines from it being turned down to a taper

If it's a stopper or plug and it is not threaded wouldn't there be some kind of attachment for a ring or chain or something to grab and pull it?
Do the two protruding pyramidoid parts have holes or something like that?

Sink stopper.

Circa 1920-1958, bathtub stopper. They use to have metal chain thing threaded through do they are easy to pull back out of bath tub drains. The hollow inside and beveled edge side are also a dead give away.
