I think you might very well be right, Brian W! :thumbsup: It's almost an exact match to the nozzles I saw when I googled antique fireplace bellows. The wood looked solid to me but that may be dirt clogging the opening. There are tiny pins in the wood (one is visible in the photo), in addition to the nails, that look like they could have been used to fasten leather to the wood. I will try boring through the opening to see what happens.

UPDATE: I cleaned out the center and it was dirt, therefore, the nozzle is hollow. I conclude that Brian W is correct and I will mark this thread SOLVED! I love the helpful and knowledgeable folks on TNet!
