Not sure what it's called I'd say Chinese carving possibly ivory or could be a synthetic material.

Is there any grain to the material which would suggest it might be ivory.

I think Bruce nailed it. Have seen somewhat similar items in Chinese restaurants that had chopsticks sticking out of them. But never anything THAT impressive.

Truthfully, I was only kidding around.
I kind a thought you were right as a holder cup kind of dealeo, but for these carved bone forks instead of chop sticks is what came to my mind when you mentioned chopstick holder.
