When I was a kid I dug this up in my parent's backyard. On and off over the years I've pulled it out and tried to do more research. Today I noticed there is a somewhat similar item on ebay and it's described as an 1800’s Conestoga covered wagon tool. Has anyone seen anything like this before?

I think the best description would be along the lines of multi-function crate hammer. And apparently a “box terrier hammer” is another term for such tools

Has an integrated nail pulling slot. I think the crate hammer makes more since but that's just my opinion.

Yep,
Packing box hammer
I've got one around here somewhere, used to use it building fruit crates.

They still make nail/tack pry bars with the same slot in it and in the same general spot as your tool. Crate hammer is my opinion, as well.
