Looks like post mint damage to me. The incuse designs on the reverse seem to be punched all the way to the front. The flattened crescent on the obverse is likely due to wear rather than any sort of off striking, as the rest of the coin is well centered.

almost looks like it was hit with some sort of punch or shot with hollow point pellets or something
