Probably Chicago, IL based on the street address. It is not listed in Vacketta's Trade Tokens of Illinois and I don't find him listed in the three early Chicago city directories I have.

A search of the mormon website shows sixteen people with this last name. Fifteen were born in Pennsylvania and one was born in Chicago in 1901. None had a name begininning with C and the first listing had a C for a middle initial.
