Need help w/Elks token,City&State?

Hello everyone...i got this elks token,i can make out the B.P.O.E 612(Lodge)on the reverse say good for5 in merchandise,but no city or state! anybody know this lodge #?,city/state??,any help will be greatly appreciated!
