Key fob souvenir from San Juan Capistrano with a Saint Christopher medal combination is my guess.

Interesting that it seems to have been cast to have a strap loop cutout (like a watch fob), but instead just had a hole drilled (like for a key ring). Maybe made shortly after watch fobs fell out fashion, whenever that was? ... I'm too tired to know if that makes sense.
