Very intriging item.
I want to say some kind of a Locket...for a photo or monocle maybe, but the chain is so big.
Looks like something attached to a pocket watch?

Maybe a watch fob and chain. If it doesn't open- possibly held a fancy stone, fancy colored glass
or a compass.
