Unknown coin-like thing

Hi,can anyone I'D this for me,it is like a hammered coin(about 3/4 of a foot deep),but it has a kind of stumpy thing on the back.What is it?

A lot longer than that shell that one was a paper shell, originally they were all brass. HH
