Defect 1978 Nickel of Thomas Jefferson

Thomas Jefferson with a hair piece? No not really. Any thoughts how this might have happened? On the reverse side, part of the letters in "states" has been flattened just a bit. Not shown on the picture the coin has a very slight warp as if bent when it was punched out. Thoughts?

it's called a 'filled die' error. I've had a few wheat pennies with small filled areas, but that's the first nickel I've seen with a fill area that large
