It is aluminum and a little larger than a quarter. On one side there is a star but I can't make out anything in it. Around the edge starting at the top and going counter clockwise is "Masons Annuity/ 1900/??" The other side reads To Identity ?? Telegraph the Numbers 6685 ??(to Masons?) Annuity/ Atlanta Ga."
I can find Masons Annuity on the internet but nothing about this token. Can anyone provide me more information?

Your token is more of an ID tag issued by the annuity company. They used to issue ones that clipped on to overall straps, too. They were meant for emergency contacts in case of an emergency where the bearer was unresponsive or dead. Here is one from an insurance company:

I think I see QU__EM Lodge on one side and Atlanta on the other. I have a hunch that it is railroad related, possibly carried by a rail worker.

Edit: You might send a photo to the folks at the Annuity Museum to see if they can shed some light on your tag.
