That was pretty much a universal bumper jack from the 50s-60s. The "foot" is missing. It was a steel maybe 5x5 inch plate that the jack sat in and the tiretool was also used for a jack handle. A little switch on the side could be changed from up to down for the jacking motion.
Like oneshot said, a universal bumper jack. Had a base it fit into and would raise any car or truck generally with a strong enough bumper. When you got it too high the car generally fell back off it and some people were dumb enough to jack a car up with it, then climb underneath the car and work on it, usually for the last time...
