Permission slips and liability forms to detect private property

I am in the process of obtaining permission to detect a graduate school that has buildings on it that date back to the 1800's in an old part of town. Part of the deal is that I carry a permission slip signed by a school official with me while detecting and sign a liability waiver to absolve the school of liability in the case of injury.
Anyone have a copy of either of these forms they wouldn't mind sending my way?

Nice form. Thanks for posting it. I printed off a copy and put it away for future use.
 
If I'm not mistaken, I think we have an attorney on this forum... ;D, he could probably give you some good advise.
