It is a button cover .. either railroad or trolly related from the early-mid 1900's

That is true for the most part. However, some street trolleys in bigger cities pulled light freight cars which needed a brakeman riding on the car to keep them from punching their way through the back of the street trolley when stopping.

I remember as a kid in Baltimore, some of the street trolleys were not passenger trolleys.
