I still don't see a "screw".

Could be ornamental or is it a spring? Some kind of "shock absorber" for a watch chain or self-adjusting bracelet?

I think that DCMatt pretty much gave you the best answer's/examples that you are going to have.
Although not an EXACT match to your piece, I would agree it is a link to a watch chain.
Hundreds were made over the years in just as many styles.
Yours is just one of them.
If you don't think this is a correct ID for you piece.... What do you think it could be??
