It seems to have threads on top, but after a closer look the nerl does not start at the top, so you wouldn't be able to screw anything on this. Maybe something was over the top so the charm could swivel around. Is a piece of a necklace? Maybe a picture was on it?

This makes total sense and looks like one. It was familiar looking. I am not sure about hair inside of it difficult to see.
