Unknown medallion: Help!!

I inherited this unusual medallion from my late father about ten years ago. I’m not sure about what it is exactly, but I’m guessing some sort of religious medal. I think he told me he found it years ago in northern Michigan while walking through some woods. It is very thin, has what looks like Latin writing on it and generally very worn. The thickness is uneven and it measures 1.5”. If anyone has information on what this is and about how old it is (my father seemed to think it was quite old) I’d greatly appreciate it!

My medal looks very old: it appears to be cast bronze and the figure lying down is the death of St. Francis Xzavier in 1552. I read that he was canonized in 1622, so I’m guessing this medal dates to the seventeenth century.
