Found this $1 star note today in my change and noticed one of the serial numbers wasn't aligned properly. According to google this is caused by the serial number sequencing machine getting stuck before it could lock in place to stamp the number straight, it is known as a "Turned Digit" Error or "Gas Pump Serial Number".
Since the sole reason star notes are made is to replace bills that were accidentally produced with errors on them I'd imagine it's quite rare to find a star note with an error.
