I have had this for some time & am not sure what caused the error. All of the letters looked smashed for lack of a better word, then on the reverse the lamanation goes all the way around the coin.

my bad, but is there no one out there willing to come up with an explanation of the error?
 
I'm not sure but it looks like it might have spent a good part of its life in a clothes dryer.
